Maryland Certified Nursing Services

 

BHC Homecare LLC. is a Maryland Licensed Level 3 Residential Service Agency (RSA) providing nursing oversight and delegated clinical support services in accordance with physician orders and applicable Maryland regulations.

Services are delivered in the community setting under the supervision of a Registered Nurse (RN) and are designed to support safe, stable, and independent living for individuals receiving Medicaid Community First Choice (CFC) or other home and community-based services.

All care is provided in coordination with the client’s approved plan of service and interdisciplinary care team. Here are some of the services we provide

RN Care Assessments

 Registered Nurses conduct initial and ongoing assessments to evaluate the client’s health status, functional needs, and risk factors. The RN develops and updates individualized care plans and ensures services are adjusted appropriately based on changes in condition. 

Care Oversight

Our RN team provides ongoing clinical oversight of care delivered in the home. This includes monitoring client status, reviewing care needs, supervising delegated tasks, and coordinating with physicians, hospitals, pharmacies, and Medicaid CFC Service Coordinators.

Medication Help

 Medication services are provided under RN direction and may include medication reminders, assistance with medication routines, observation for side effects, and monitoring for adherence to prescribed regimens.

Medication administration may be provided only when appropriate, within scope, and as delegated by the RN in accordance with applicable regulations and the approved plan of care.

Health Monitoring

 Care staff and nursing personnel may monitor and document vital signs and other clinical indicators as directed in the care plan. This supports early identification of changes in condition and timely communication with the RN and healthcare providers. 

Chronic Care Support

 Nursing oversight is provided for clients with chronic health conditions such as diabetes, hypertension, COPD, and heart failure. Services focus on monitoring symptoms, identifying changes in condition, reinforcing provider instructions, and supporting continuity of care.

Post-Hospital Care

 We provide RN-directed monitoring and coordination following hospital discharge or rehabilitation stays. Services support a safe transition back into the community by ensuring care plan follow-through, medication reconciliation support, and communication with healthcare providers.

Wound Care

 

Wound care services may be provided under RN supervision and physician orders. Services include assessment, monitoring of wound status, and dressing changes performed by qualified nursing personnel as appropriate within the care plan.

Catheter Care

 Care may include catheter maintenance and monitoring, hygiene support, and observation for signs of infection or complications, provided under nursing direction and within the established care plan. 

Caregiver Ed.

  Education is provided to clients, family members, and caregivers to support safe care at home. This may include instruction on medication routines, chronic disease management, safety practices, mobility support, and recognition of changes in condition that require clinical attention. 

RN Staff Supervision

 All direct care services are provided under the supervision of a Registered Nurse in compliance with Maryland Residential Service Agency requirements. The RN ensures staff are trained, delegated tasks are appropriate, and care is delivered according to the client’s plan of service.

 Our focus is to support individuals in maintaining independence, safety, and dignity in their home environment while reducing preventable hospitalizations and supporting continuity of care within the Maryland Medicaid community-based service system.
